This form serves as your formal request to appeal the expiration of your W.I.T.S certification due to extraordinary circumstances. The process takes up to 45 days as we gather information and input from the student database on your past records in renewing. The fee for this review is $50.00. If the review is upheld in your favor due to some fault of the W.I.T.S. staff then you will receive a full refund of the fee. Otherwise, the fee to be reviewed and considered for the additional extension past the policy grace period of 60 days is non-refundable.
Please note: This form MUST be completed and be accompanied by any supporting documentation pertinent to your case. Any form received without additional documentation will be rejected outright and noted as such on our return letter. Completed forms will be processed within 45 days and you will be contacted with the results of your review by email from the W.I.T.S. Certification Director. The results of the findings are nonnegotiable.
