Americans with Disability Act (ADA) accommodations needed?* * 

Do you need Americans with Disability Act (ADA) accommodations? If so, please complete this form and send all ADA official document forms to witstesting@witseducation.com so we can share them with the hosting college for assistance. Delays in receiving the paperwork for verification can delay your test being delivered on time.

COVID-19 Notice * 

We care about our student trainers, teachers and testers. Therefore, we are requiring masks to enter the gym or college location at the assigned time/s.

Cancellation Policy * 

I confirm that all cancellations need to occur a full 72 hours prior to the scheduled exams in order to receive a refund of my testing fees.

Terms and Conditions:

  1. The gift testing voucher retest fee is $75.00 per exam. (written knowledge exam or the practical skill competency exam)
  2. The NO gift testing voucher candidates retest fee is $150 per exam (written knowledge exam or the practical skill competency exam)
  3. Payment must be received 45 days prior to re-test date in order to guarantee your requested retest weekend.  (No refunds.)
  4. If you cannot attend your scheduled retest, there will be a $50 re-scheduling fee. Rescheduling must be done 48 hours prior to the testing date during business hours. Office hours:   Monday – Thursday 8am-6pm EST. Any student who fails to appear for their scheduled retest time, without proper notice to reschedule, will forfeit their original  retest fee.
  5. Retest offer is only good for 12 months from your original class sign up date.  Retesting is limited to 2 attempts per student.
  6. Students will be notified no later than one week prior to retest date of time, day, location and tester. A return message must be left confirming the details. (Please provide your  most up to date phone number and email address)
  7. Retest locations to be determined based on area registrations.
